Your first visit
In your initial visit you can expect to review your medical history, discuss your current health status, and assess your goals for wellness. You will work together to develop a plan of care which generally starts with baseline interventions and data collection.
Once your baseline is established, we will begin digging even deeper to reach your personal goals of optimal wellness. Your nurse will reach out to you within a week of your initial visit to review your plan of care and address any questions you may have.
Membership
Your PHW membership is designed to give you full-spectrum support — from preventive screenings and lab panels to discounted services across the clinic and movement center.
Everything listed is included with your monthly membership at no additional cost, unless otherwise noted.
Types of Visits
We’re here to support you with any of your care needs, and the visit types below can help guide how to schedule your care.
Annual Wellness Visit
Review Member Labs, Assess Pillars, Set Goals and Road Map for the year. These should be scheduled as an in-office visit once annually.
Office Visit
Follow Up/Check-Ins, Lab Reviews. These can be on-site, over the phone, or via video conference.
Acute Visit
As needed work-in spots for acute illness, injury, or concerning symptoms. Times are held daily to take care of acute patient needs. These visits may be with a provider other than your primary provider. These can be on-site, over the phone, or via video conference based on the situation.
Wellness Coaching Visit
Check-ins with Wellness Nurse to receive coaching support for lifestyle goals or to clarify or adjust your plan of care. Can be set up as phone visits, on-site visits, or even portal message check-ins.
Billing
New patient memberships are $120 per month for the first year and $100 per month thereafter. Payments come out monthly on the day you signed up. Your first month is included in your new patient visit fee.
We do not require a membership contract. Membership cancellation requires a 60-day notice for continuity of care and can be processed through the patient portal or by calling one of our clinic locations.
We are out of network with all insurance companies. If you’d like to submit expenses to insurance, we can provide itemized receipts with diagnosis codes for labs, IVs, office procedures, and more.
